Brentford's Lewis MacLeod scored a stoppage-time equaliser to prevent Albion a win in this mid-season Championship clash.
Substitute MacLeod headed in Emiliano Marcondes' cross, earning the Bees a point in a game dominated by the Baggies.
West Brom missed numerous gilt-edge opportunities before Harvey Barnes gave them a second-half lead, firing his eighth goal of the season into the top corner from 12 yards.
Albion boss Darren Moore was left scratching his head at how his side were not three or four goals clear at the break, with Jay Rodriguez slicing an early chance over an open net and Hal Robson-Kanu heading wide from six yards.
Matt Phillips also smashed a fierce 30-yard strike against the crossbar - one of West Brom's 16 first-half shots.
West Brom 4 Brentford 0 - 12 August 2003
On a warm summers evening in the West Midlands, Albion ran out comfortable 4-0 winners against Second Division Brentford in the first round of the League Cup.
Rob Hulse and Bernt Haas marked their Hawthorns debuts with goals.
Hulse, a £750,000 summer purchase from Crewe, gave Gary Megson's side the lead in the 18th minute with a low drive following an impressive run and cross from midfielder Jason Koumas.
And Hulse was in the thick of the action again just before half-time forcing a foul from Brentford's goalkeeper Paul Smith, who was shown the red card for serious foul play.
It took until 73 minutes for the Baggies to make their domination count, a Neil Clement cross enabling Haas to double their advantage, with Hulse adding a third, heading in another Koumas cross.
Scott Dobie completed the rout with a shot from close range in injury time, after a pass from new signing Artim Sakiri.

Brentford advanced in the League Cup 4-2 on aggregate after a convincing second half display against West Brom.
After a cagey first half, Jamie Bates handed the Bees the advantage with a looping header over Alan Miller.
Charlie Oatway doubled the hosts advantage moments later when he prodded home an inviting cross from close range.
Despite Albion pressure, Brentford held firm and rubber-stamped their passage through to the next round when Lloyd Owusu calmly finished past Miller from close range.

In the first leg of this first round League Cup tie, Albion narrowly got the better of Brentford thanks to two first half goals.
Irish striker Micky Evans handed the Baggies the lead on the 35th minute with a tidy finish.
Lee Hughes, enjoying his second season with the Baggies, doubled the hosts advantage shortly before the interval.
Despite threatening to put the tie out of sight for the Bees, Brentford handed themselves a vital lifeline with moments remaining courtesy of Kevin Rapley.
It would set up a mouth watering second leg in the capital.

Taylor latched on to a looping through ball to calmly finish past the visitors keeper.
Wayne Fereday douboled the Baggies advantage late on with a fantastic solo effort seeing him evade numerous Brentford defenders before poking home past the helpless keeper.
