Charlie Lakin climbed off the bench to secure the Saddlers' place at Wembley for only the second time in their history when he headed in Albert Adomah's cross in the 81st-minute.
Levi Amantchi had missed a glorious chance just moments before and Chesterfield midfielder Jenson Metcalfe had a goal ruled out for handball earlier in the second half.
Armando Dobra equalised for Chesterfield in the fourth minute of stoppage time but Levi Amantchi scored a dramatic 95th-minute winner.
Walsall will face either Notts County or AFC Wimbledon on Monday, May 26, in their first play-off final since 2001.
Mat Sadler remained unchanged across the 18 from the 2-0 win at the SHM Group Stadium in the first leg.
Meanwhile, Chesterfield made three changes as key wide man Armando Dobra returned following his cameo from the bench on Sunday.
Walsall made a rip-roaring start and had the first chance inside the opening three minutes when Ryan Boot thwarted Jamie Jellis at his near post.

Chesterfield eventually settled on the ball but Walsall were relentless in the press, chasing their opponents like a pack of dogs.
The visitors had no other option but to recycle the ball across the back-line and Walsall successfully prevented them from building any rhythm during the first half-hour.
Dobra looked the Chesterfield player most likely to inflict damage but Tommy Simkin saved comfortably after he unleashed a floated effort from distance.
The Saddlers almost caught Chesterfield on the counter-attack when Liam Gordon pounced on a loose touch by Metcalfe.
Gordon had options left and right but eventually fed Jellis on the edge of the box. He managed to work his way into the box but his effort was straight at Boot.
At the other end, Chesterfield had their best chance of the first half through Tom Naylor but Taylor Allen made a crucial block to divert his goal bound header behind.
Bim Pebble started to threaten with his runs in behind, and got the better of Harry Williams on a couple of occasions, but Allen was alert to the danger once again with a sweeping last-ditch challenge.

Metcalfe drove a low shot into the bottom corner just eight minutes after the restart but the linesman quickly raised her flag after spotting a handball.
Sadler used his bench for the first time just past the hour-mark when Brandon Comley and Levi Amantchi replaced Alfie Chang and Jamille Matt.
Substitute Jack Sparkes smashed a fierce effort towards goal at the back post but Nathan Asiimwe made another vital block to keep the Spireites at bay.
Amantchi had a glorious opportunity to give Walsall the lead when Hall squared it to him at the back post but Boot swooped across his goal to deny him.
Walsall would be celebrating nine minutes from time when the two substitutes combined to great effect. Adomah escaped down the right and lofted an inviting cross to the back post where Lakin got in front of his marker to head home.
Amantchi flashed another low effort across goal and wide of the far post. Dobra got Chesterfield back on level terms with a stoppage time effort but Walsall immediately raced up the other end where Amantchi headed into the bottom corner at the back post.
Fans spilled from the stands in the aftermath and there was a lengthy stoppage as the referee waited for the Walsall supporters to vacate the pitch.
