Alex Dickenson was travelling on a blue Yamaha motorbike at about 7.15pm on November 6 when he was involved in a collision with a white Mercedes car on the eastbound carriageway, between Ventura and Marlborough Way.
Damage was caused to both vehicles.
Debris from the collision hit a white Vauxhall Adam travelling in the opposite direction, causing the driver, a woman in her 50s, to stop.
A white Scania HGV was then involved in a collision with the stationary Vauxhall and Alex Dickenson.
Sadly, he suffered serious injuries and died at the scene.
In a tribute to Alex, who was from Tamworth, his family said: “Our Alex, so kind, so warm, so gentle, so funny. He will be forever loved and missed.
“All our love.”
The driver of the Vauxhall suffered serious injuries and was taken to hospital where she continues to recover.
The driver of the HGV stayed at the scene and is assisting police with the investigation.
A spokesperson for Staffordshire Police said: "We’re keen to speak to anyone who saw what happened on either carriageway or those with CCTV or dashcam of the area at the time. Call 101, quoting incident 611 of 6 November, or email collision investigators directly using ciu@staffordshire.police.uk"
